Accessing Your Medical Records

You have a right to access your health records. Requests for access to health records can be made directly to either Bethesda Mount Hospital or Bethesda Claremont Hospital, depending on where you were treated.

What Can You Request?

• You can request records for yourself, or on behalf of another patient. You will need to provide authorisation in writing from that person and certified identification for you and the patient, allowing you to access their health records/information.
• If you are applying for health records/information of a deceased patient, you will need to provide evidence that you are the Executor of the Will. If more than one executor has been nominated, all parties will need to sign the application form and provide certified ID as applicable.
• You can request amendments to your health record, unless there is a reason under the Privacy Act 1988 (Cth) or other relevant law to refuse such access or refuse to make the requested changes.

How Much Does It Cost?

Under the policy there is no fee for an application, but an administration fee may be charged, for processing which can include photocopying, retrieval, preparation, and delivery of records.

How Do I Apply?

You may apply by downloading and completing the form below. Applications cannot be processed if there is missing information or identification.

If you would like discharge summaries/certain dates/events only, we suggest you apply only for the relevant information by including dates or information on your application form. This will assist us to process your request in a timely manner.

Once you have completed the form, return it to the hospital where the patient was treated.
